Onsite vision services MI 2025-13
Performance date: November 1, 2025 through October 31, 2026
Quotes are due on Friday October 17, 2025 by COB

Submitted bid will show at minimum breakdown as follows:
On-site service scheduled as needed but not to exceed 12 occurrences in 12 months.
Vendor to bring necessary equipment for vision exams, glaucoma exams, and center will provide the space to set up and work.
Vendor will identify how many students they can see in a 8 hour day.
Vendor to certify that they will bill student insurance when available.
Vendor to quote on the cost of the vision exam and basic glasses.
Vendor to identify any additional fees or costs associated with their service.
Freight, fuel surcharge or handling charge, if any, must be noted on bid or charges will not be paid if invoiced.

Eye Exams
Glaucoma Exam
Glasses as needed
Eyeglass case
Adams and Associates, Inc., contracting for the U.S. Department of Labor for the operation of the Miami Job Corps Center – a student residential training facility located at 3050 NW 183rd St Miami, FL 33056 is currently soliciting written bids for this service.
