Triages patient calls and facilitates patient inquiries and requests.
Tracking patients’ hospitalization status by communicating with facilities and social work teams.
Communicating professionally and efficiently with physicians to help the clinical team obtain and clarify orders
Facilitates communication between contract vendors and clinical teams and obtains clinical documentation timely
Facilitates the ordering and delivery of supplies to patients' homes.
Completes the required documentation to ensure that all activities are appropriately noted.
Participates in data collection for patient surveys.
Assist the clinical management team in follow-up with nurses regarding documentation and compliance issues.
Performs all related duties to enable the efficient operations of the agency.
Performs all clerical duties necessary to support the efforts of the clinical teams.

Education, Knowledge, Skills and Abilities Preferred
Excellent customer service skills.
High School Diploma.
Experience in health care or a related field.
Effective verbal and written communication.
Sound judgment and critical thinking abilities.
Excellent organizational and interpersonal skills.
Computer literacy.
Bilingual a plus.
