The  Nurse Clinician Coordinator  reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.The Nurse Clinician Coordinator provides direct patient care and is accountable for the assessment, planning, implementation and evaluation of a plan of care for designated patients. The Nurse Clinician Coordinator ensures that quality outcomes are achieved and demonstrates exceptional clinical practice.Responisbilities:Provides evidence based patient care and is accountable for the assessment, planning, implementation and evaluation of a plan of care for designated patientsFacilitate timeliness and access to care and coordinate complex care processes throughout the Northwestern Healthcare System.Coordinates patient care across the continuum of careAssists patients in interpreting care recommendations as needed.Assesses patient and family learning needs, assists in the development of evidence based educational material and provides patient and family education.Evaluates patients’ physical and emotional recovery from procedures and strives to promote patient wellness and comfort.Serves as a resource for patients and their families, and helps to coordinate resources as needed.Monitors quality of care, as well as patient and physician satisfaction, through follow-up discussions and assessments
