Resident Care Coordinator

$23.00 per hourTeam lead role over Assisted Living CNA team membersSupport HWD by managing Assisted Living team members.Schedule Assisted Living team members.Participate in Care Plan meetings with families and residents and manage ISP’s as resident needs change. Promote all company policies by ensuring SLC communities are providing the best care possible for our residents.Participate in weekly at-risk meetings with HWD, ED, MCC/ MCD to ensure leadership team is aware of all resident needs.Review and implement all necessary processes for optimal operational efficiency such as- shower schedules, laundry schedules, caregiver assignment sheets, etc.
