1.General job duties. Types of procedures required. Will treat patients in the home setting doing Initial evaluations, routine visits, reassessments, and discharges.
Equipment utilized in the unit/dept. (Is there computerized charting, medication systems, etc.)
Electronic Medical Record using Epic
Required skills and minimum years of experience.
1 year ST experience; Home Health Experience Preferred; OASIS experience preferred
4.Specific licensures and certifications/registrations required. (ARRT, CRT, BLS, ACLS, etc)BLS; Illinois License for Speech Language Pathology5.Shift or schedule requirements. (Will the employee ever work alone? Is call required? If so, how often? Are weekends required?)1st shift. Monday-Friday 8-1630. Will work independently going to patient's homes. No on call, no weekends.
