Position Summary:
Coordinates resources for mental health and addiction services for patients, families, and caregivers upon assessment, during hospitalization, and at discharge. Provides clinical expertise, coordination of services, and acts as an advocate to assist the patient population in navigating throughout internal and external systems in order to facilitate the treatment process and continuum of care. Will serve as a liaison and resource to patients and staff. Helps to eliminate barriers to care and ensure timely delivery of services. As a member of the multidisciplinary team, the Behavioral Health Patient Navigator contributes to quality improvement initiatives regarding pediatric mental health and addiction care.
Qualifications:
EDUCATIONAL REQUIREMENTS Bachelor's Degree in Social Work or related field ADDITIONAL REQUIREMENTS Crisis Prevention Institute (CPI) training within 3months, and Basic Life Support (BLS) (AHA) within 30 days SKILLS AND KNOWLEDGE Able to tolerate stressful situations with families, caregivers, and patients. Ability to multitask and float between departments as necessary.
Essential Functions:
Coordinates and manages the delivery of behavioral health services, including therapy.
Conducts comprehensive assessments of clients to understand their mental health history, current needs, and treatment goals.
Assesses and documents patient and family education as applicable
Acts as a liaison and works with other youth services to coordinate and/or retrieve information pertinent to patient care.
Identifies patient and family needs for support
PERFORMANCE IMPROVEMENT FOCUS: Assists in Performance Improvement activities for area.
Actively promotes and achieves customer satisfaction (patient, family, departments and physicians)
Assists in completing chart audits and compiling other data to evaluate performance
Compiles and assists in analyzing intake, utilization and patient flow data
Generates ideas for improving processes, treatment, and patient care
Participates in Root Cause Analyses and assists in development of action plans and implementation of action plans
Key Accountability Participates in in-service and competency training. Prepares special reports as needed on activities, and work group activities.
Follows all policies and procedures
Complete clinical documentation and other forms of communication in a timely manner All documentation is completed the same day of service and accurately reflects the reason for contact, interventions used and outcomes Reports any critical or unusual situations and completes incident reports as required Interact and communicate with law enforcement, medical staff and other community providers effectively Communicates effectively in-person, through email; responds in a timely manner to voicemails
Works in a highly collaboratively manner with therapy clients' assigned nurses, clinicians, and other service providers, within and outside facility.
Builds awareness among the healthcare team, patients, and families regarding mental health and addiction services
Key Accountability Meets clinical productivity standards Meets compliance standards for all documentation and clinical charts.
Facilitates appointment scheduling processes, including initiating the necessary initial paperwork as a part of the coordination effort.
