Location: Holy Cross Health has two hospitals and four healthcare centers all a short driving distance from Washington DC and Baltimore, MD

Description:

Full-Time Days 8:00a-4:30p

Under the direction of case management staff, the Discharge Planning Assistant will provide quality patient care considering age specific, developmental, and cultural needs. Demonstrates unit/area designated competencies.

Work closely Social Work/Case Manager, physicians, members of the interdisciplinary team and external agencies when gathering or exchanging appropriate patient information.

Responsibilities:

Set up transports from hospital to rehab facility using transportation services provider.

Obtain prior-auth for patients being transferred to Skilled Nursing Facilities (SNF)

Follow up on referrals, missing patient documentation and checking on the bed status at SNF.

Facilitates referrals to community agencies as directed by Social Worker/Case Manager via phone and web-based applications (EPIC)

Communicates with SNFs (liaisons and admission teams) to facilitate appropriate patient placement taking into consideration: a. patient/family choice, b. patient clinical condition/needs and c. patient insurance coverage.

Determines need for and obtains authorization from the patient’s insurer for post-acute care including SNF services and ambulance transportation.

What you will need:Required:

Education: High school diploma or GED required

Preferred:

Education: Some college preferred or equivalent experience including knowledge of medical terminology.

Experience: Minimum one to two years’ work experience in health care setting preferred.

Analytical skills necessary to facilitate transfer of the patient.

Organizational skills to assess urgency and complexity of referral and to facilitate appropriate and timely patient transfer.

Strong attention to detail, ability to prioritize work and adapt quickly to changes and interruptions in a fast-paced environment to meet deadlines.

Proficient with MS Office, working knowledge of Excel, typing, data entry and transcription skills including accurate spelling and grammar.
