Job Description

The Patient Navigator supports patients in active cancer care within the Tufts Cancer Center. He/she will work closely with a multi-disciplinary care team (including, but not limited to oncologists, nurses and social workers to ensure optimal patient care. Responsibilities include attending clinic visits at patient's request, coordination of patient appointments/follow-up, identifying and reducing barriers to care, assisting with non-medical needs (e.g., coordinating with social work for transport needs). The incumbent may be called upon to assist the Medical Director of the Patient Navigation Program and a Principal Investigator within the Institute of Clinical Research and Health Policy Studies in writing medical and cancer-related scientific reports, grant applications and manuscripts in English and Chinese. These documents will relate to the impact of cancer and cancer care on persons of Chinese origins. This incumbent will perform functions both independently and with others.

Responsibilities:

  • Responsible for assisting patients on active cancer treatment: serving as medical/cultural liaison between family and clinical care team, acting as patient advocate, assisting with care coordination and scheduling, connects patient with other hospital resources.
  • Supports the care team by communicating with patients to follow-up after appointments and relaying information from the patient to their clinical care team.
  • Builds and maintains collaborative, team relationships with colleagues and patients.
  • Assist with writing of research progress reports for federal and foundation grants, annual reviews, and/or manuscripts.

Qualifications

  • Bachelor's Degree or equivalent required.
  • 3+ years of experience required.
  • Demonstrated oral and written fluency in both English and Chinese (at least 1 dialect).
  • Demonstrated proficiency with MS Office required and the ability to learn new software (e.g., electronic medical record systems) as needed.
  • Proven meticulous attention to detail.
  • Superb organizational skills and ability to efficiently prioritize and complete workload within time constraints.
  • Excellent judgment, professionalism, and tact required for interaction with researchers, staff and patients.
  • Ability to handle confidential information according to regulations including study subject enrollment, payment records, confidential correspondence and HIPAA.
  • Proven ability to highly self-disciplined, motivated and capable of self-direction within a defined scope of work.
  • Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging process, human development stages and cultural patterns in each step of the care process.
  • Ability to work independently and as part of a team process.
  • Ability to assist with the preparation of grant applications, reports, manuscripts, memos, protocols, and publications in both English and Chinese.
  • Normal office work environment, Cancer Center clinic space.
