What You'll Do:

Empower Young Families: Work closely with youth and young parents, guiding them through transitional care and aftercare outreach. Your role will be central in helping them build a brighter future.

Coordinate & Communicate: Oversee intake processes, ensure smooth transitions, and maintain open lines of communication with clients, the Young Parent Living Program, and other key stakeholders.

Create Impactful Plans: Develop and review treatment plans, safety plans, and assessments, collaborating with clinicians and DCF social workers to ensure comprehensive support.

Foster Independence: Lead and facilitate programs that boost life skills, parenting techniques, and job readiness. Support young parents in achieving their goals, from budgeting to finding housing.

Be a Role Model: Provide leadership and serve as a positive example for young parents and your team. Your influence will inspire and motivate those you work with.

Innovate & Educate: Conduct in-house trainings, stay updated on best practices, and contribute to our program’s continuous improvement. Your expertise will drive our mission forward.

Who You Are:

Experienced Professional: You have a Bachelor’s degree in psychology or a related field and at least two years of experience in a residential setting. Your experience has honed your ability to manage complex cases with empathy and efficiency.

Dynamic Communicator: Your communication skills are top-notch, and you can collaborate effectively with clients, colleagues, and external partners.

Organized & Detail-Oriented: You excel at managing multiple tasks, maintaining accurate records, and ensuring compliance with regulations.

Adaptable & Resilient: You’re flexible with working hours and can handle the challenges that come with working in a fast-paced, evolving environment.
