We at Ebenezer Ridges Care Center- Ebenezer are searching for a Social Service Designee who assists the Social Service department in meeting the needs to long term and transitional care unit residents and families through various tasks and activities completed under the supervision of the Social Service Director.This social work support position includes;

80 hours every two weeks

Full time; weekday shift

Assists with coordination of admissions as appropriate

Assists social work staff with caseload assignments while establishing relationships with referral sources

Assists with marketing and outreach as appropriate

Complete MDS and associated assessments per regulations

Assists with one-to-one and group counseling as appropriate

Assists with discharge planning

Assists in Vulnerable Adult investigations and completes necessary documentation and reports as appropriate

Required:

0-1 years experience in related field

Preferred:

2-3 years experience in similar field; preferred long term

Bachelor's Degree from an accredited school of social work

Licensed by the State of Minnesota Board of Social Work
