Licensed Practical Nurse / LPN

The LPN is responsible to provide quality health care services to persons with disabilities within the standards of community-based health care. The multi-site nurse maintains continuation of the nursing process for each individual authorized to receive nursing services under the supervision of the registered nurse.The LPN assists the supervisor and the nurse supervisor in training and orientation of staff, provides leadership and guidance to staff in the supervisor’s absence, coordinates the health services of individuals under the supervision (as defined by the Minnesota Nurse Practice Act) of the nurse (designated nurse and/or health services coordinator), performs specifically delegated medical and nursing actions, as determined by the physician and/or nurse.
