DIABETES TECHNICIANREGULAR/FTNON-EXEMPT - LEVEL 4SALARY: $14.75GENERAL DEFINITION: The purpose of the position is to assist in activities of the program to promote diabetes awareness, prevention and maintenance of diabetes and the benefits of healthy lifestyle.SUPERVISION RECEIVED: Work is performed under the general supervision of the Health Education Coordinator and the direction of the Program Director.SUPERVISION EXERCISED: None.DUTIES:Assist with program activities to promote diabetes education programs for self-managing diabetes participants.
Monitor monthly work staff schedules to ensure effective scheduling.
Submits monthly activity reports to communicate achievements and ongoing projects.
Assist with focus groups and help facilitate various diabetes education sessions to improve the quality of life of healthy lifestyle and diabetic participants.
Perform cooking education classes to age appropriate audience to incorporate good, healthy cooking techniques to help manage their blood glucose.
Assists at outreach activities related to nutrition education and physical activities to improve diabetes self-management education and support services to reach healthy lifestyle goals.
Attend meetings with other health programs to coordinate outreach events.
Create educational materials such as brochures and posters to assist with public service announcements.
Conduct inventory monitoring of all medical supplies and exercise equipment.
Health screening clinics, health fairs and events, including but not limited to: YLD, outdoor play, weight loss programs, walking programs, softball league games, Little league games, Lifestyles balance curriculum, Slum Dunk Diabetes basketball and physical fitness sessions.
Conduct follow up assessments during various interventions such as (non)competitive activities, challenges to improve diabetes outcomes.
Perform monthly fitness activities at Fitness Place; providing organized and structured physical activity sessions including but not limited to spinning, weight-training, walking, running and other instructor-led group classes.
Develops and implements an incentive program for participants to help educate and support participants to take an active role in managing their diabetes.
Track attendance and progress of participant’s by entering data into the database in a timely manner and provide analysis reports, as needed.
Assist with obtaining participants health measurements such as height, weight, body mass index (BMI), glucose, blood pressure, and A1C including health questionnaires and other appropriate data, as needed.
Attend training sessions and conferences related to at risk populations of diabetes to improve support of participants and overall operation performance.
Provide program information and refer clients to appropriate programs and/or staff for continued services.
Provide caseload management with effective one-on-one coaching with motivational interviewing, goal setting and progress tracking to provide prevention awareness to all participants.
Maintain collaboration with local agencies to emphasize effective facilitating active learning and new lifestyles with people living with diabetes.
Manage partnerships with local agencies and other community health programs to significantly improve the health status of the community.
Makes referrals to other agencies as needed and appropriate and provide case management where appropriate.
Partner with community to promote and support diabetes prevention activities.
Maintain communication with participants and other agencies of challenges to provide a more feasible solution to participants.
Perform other duties related to the position as assigned.
MINIMUM QUALIFICATIONS:KNOWLEDGE, SKILLS AND ABILITIES: Knowledge of diabetic education services. Knowledge of Diabetes Mellitus and the complications and co-morbidities. Knowledgeable of OSHA requirements. Skilled in tactful and diplomacy in interpersonal interactions. Skilled in understanding participant education needs by effectively sharing information with participant and families. Skilled in operating a computer and software applications, including excel, access, and online databases. Ability to read and write in English to sufficiently maintain accurate records of customers. Ability to communicate efficiently and effectively in English and Zuni. Ability to maintain confidentiality and HIPPAA regulations. Ability to work extended hours and various work schedules. Ability to travel long distances for training throughout the nation. Ability to work independently and meet strict time lines. Ability to analyze problems, project consequences, identify solutions, and implement recommendations. Ability to establish and maintain good working relationships with co-workers, tribal employees, outside agencies, and the general public. TRAINING AND EXPERIENCE: High School Graduate; with two (2) year of experience in public service, health programs and/or community-based nutrition or diabetes programs or related experience that demonstrate the ability to perform the duties of the position. Preferred training in Chronic Disease Management or Community Health Worker.LICENSE AND CERTIFICATES: Valid New Mexico Driver License with no DWI/DUI convictions within the past three (3) years. Driving is an essential part of the duties.PHYSICAL DEMANDS: The work requires minor exertion such as short periods of standing, walking over rough uneven surfaces, some recurring bending, crouching, stooping, stretching, reaching, or similar activities. Occasionally lifts heavy objects (over 50 pounds). Job may require recurring lifting of lightweight objects with infrequent bending or stooping alternating with the lighter activities. Job requires minimal agility and dexterity. Occasional exposure to objectionable conditions or variations such as those found in variable weather conditions or light industrial settings.SPECIAL WORKING CONDITIONS: Position is subject to Pre-Employment drug testing. Position is Safety Sensitive and will be subject to random drug screening for duration of employment. Must undergo and successfully pass a thorough background check and FBI Fingerprint check. May be required to work beyond the regular 8-hour workday.
