Req Number 120833FlexStaff is seeking a Nurse Case Manager/Staff Nurse to join our client's team, a leading organization that provides housing, services and programs for individuals with intellectual and developmental disabilities.This is a direct hire, permanent, full-time position in Brewster, NY in a Day Habilitation facilitySchedule: Monday-Friday, 9am-5pmThe Nurse Case Manager will coordinate and provide nursing care to individuals with intellectual and developmental disabilities (I/DD) enrolled in the day habilitation program.Responsibilities
Coordinate the delivery of health care services for adults with disabilities residing in residential programs;
Be responsible for performing nursing assessments and diagnoses;
Work in conjunction with other members of the interdisciplinary team to achieve positive health outcomes.
Requirements
NYS RN License, BSN preferred.
Prior nursing experience and/or experience working with persons with developmental disabilities preferred.
Excellent oral and written communication skills.
Able to work flexible hours and/or days.
