Lake Ridge Care Center,a 5-star rated, 56-bed senior care campus, is hiring an analytical Nurse (RN or LPN) to join our team as a Grant Manager/Assessment Nurse. In this new and innovative position, you will utilize clinical skills, competencies, and best practices to enrich accurate resident assessments and impact the quality of life for residents we serve. This position requires you to be hands on and actively engaged with our residents. For your dedication, we are offering a $2,000 bonus! Responsibilities:

Display a courteous attitude and respect for all residents, families, and staff.

Lead the MOVE FORWARD Teams and provide direction, support and resources as required.

Effectively use program tools including assessments, audits, educational resources and technology to improve resident care and quality of life.

Complete and analyze program reports.

Execute on-site program initiatives within the community.

Provide program education to staff, resident and family members.

Perform additional tasks as assigned.

Requirements:

Must have a current MN RN licenseand be in good standing.

Must be able to travel locally as needed up to 10% of the time.

Proven leadership skills to lead and work with staff, residents and families.

Excellent interpersonal, communication, and organizational skills.

Ability to manage multiple tasks/projects and deadlines.

Understand, interpret, and utilize data to improve resident care.
