What you'll do The Licensed Social Worker is an active member of the Nephrology/Dialysis treatment team. The licensed social worker is responsible for the therapeutic relationship that includes addressing the complex bio-psychosocial needs of the patient. The scope of practice includes but is not limited to comprehensive assessment of those needs, diagnosis, treatment planning, advocacy, and resource identification and linkage. Clinical social work in the outpatient setting includes using evidence-based therapies to address the bio-psycho-social aspects of patient care such as adjustment to illness issues, mental health, co-occurring disorders, identifying and reporting child and elder abuse and neglect, domestic violence and complex care management (when appropriate). Adherence to established Conditions of Coverage for the Center for Medicare & Medicaid services (CMS) for End Stage Renal Disease (ESRD) dialysis clinics. Core Social Worker Functions (applies to all Social Workers) Participation in departmental rounds or team meetings as required Comprehensive bio-psychosocial assessment of patient Treatment planning to address issues identified in comprehensive assessment Resource linkage and advocacy with appropriately identified resources Healthcare decision making and planning for Advanced Directives Participation in departmental objectives and assignments Completes psychosocial assessment and long/short term care plans Completes Quality of Life Survey (KDQOL) Maintains Quarterly notes and updates as needed Modality Training Depression Screening Vocational Rehabilitation Provides education on lifestyle, psychosocial issues and reinforcement Provides emotional support Provides patient with support and or referrals for: adjustment to illness, alcohol/chemical dependence, grief/loss/bereavement, financial/insurance issues, and compliance to treatment Provides evidence based therapeutic interventions Performs acute/crisis evaluations and coordinates psychiatric hospitalization as needed Provides psychoeducation to patients and families Functions as a member of Dialysis Interdisciplinary Team Facilitates referrals for medication evaluations, inpatient psychiatric hospitalizations, and community support services Communicates with patient and other treating providers to coordinate treatment plan goals and objectives Active participant in departmental rounds, team meetings, patient care plans, and QAPI/Core Team Meetings as required Treats patients, colleagues, and other staff with cooperation, compassion and respect Conducts self in a manner consistent with ethical standards of practice in the field Collaborates with outside agencies, such as CPS/DSS Performs similar or related duties as requested or directed What we need from you Education: Master's Degree Social Work, required Experience: Prior related experience working in behavioral health setting such as an OMH or OASAS licensed clinic, preferred Licensure/Certifications: Current NYS License as LMSW, required Skills: Curriculum/Plan Development Clinical Skills Analysis & Evaluation Computer Skills Multi-Tasking Interacting with People Problem Solving Communication Physical Requirements: Medium Work - Exerting up to 50 lbs. for force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects Occasional grasping, pulling, pushing, reaching, stooping Frequent typing/clerical/dexterity, hearing, repetitive motion, standing, talking, visual acuity (color, depth, perception), walking Constant seeing/monitor/computer use, fit testing Hazards: Occasional bodily fluids/bloodborne exposure Working Conditions: Occasional noise, odors Travel: Extensive Travel - You should expect to travel every week Total Rewards Bassett Healthcare Network’s commitment to our employees includes benefit programs carefully designed with the needs and values of all our employees in mind. Pay Range Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. Pay Range Minimum: USD $66,556.32/Yr. Pay Range Maximum: USD $99,834.49/Yr.
