Case Manager
Responsibilities Include:
Conduct intake interviews
Guide individuals and families in need through critical social service systems
Create personalized service plans
Find and refer clients to community resources from housing to healthcare
Handle documentation including Medicaid status and program eligibility
Maintain detailed client files and ensure proper follow-up through ongoing assessment
Maintain client files
Strong interpersonal skills
Sympathetic, empathetic
Good communication
Reliable, hard worker
Pay $22-$23
BA Degree a plus
Temp to perm
