RN Case Manager – Hospice (4 Permanent Roles)
Salary: $110,000–$112,000 base (additional differentials available)
Additional earnings offered for:
$1,800 – BSN, $300 – MSN, $1,000 – Spanish bilingual
Schedule: Full-Time
Setting: Field + Office Based | NYC | Home Visits Required
The RN Case Manager is responsible for the full clinical oversight and case management of an assigned hospice caseload. This includes coordinating interdisciplinary services, implementing the Hospice Plan of Care, and conducting ongoing physical, emotional, and psychosocial assessments to support comfort, dignity, and quality of life for both patients and families.
Key Responsibilities
Manage the full scope of hospice care for assigned patients.
Conduct comprehensive physical and psychosocial assessments.
Develop, review and modify the Hospice Plan of Care based on patient needs and goals.
Provide patient/family education regarding disease progression, medications, ADLs, and end-of-life processes.
Maintain timely, accurate clinical documentation in NetSmart/EMR.
Coordinate and communicate updates with the interdisciplinary hospice team.
Ensure safe home conditions and provide ongoing, documented safety assessment.
Complete medication reconciliation at each visit; obtain refills and adjust orders with MD when needed.
Utilize clinical monitoring scales at every visit and follow up accordingly.
Perform EMR documentation during visits and complete required synchronization twice daily.
Attend IDT meetings weekly within the first week of admission and every two weeks thereafter.
Review and complete Recertification PGIs 2–4 weeks prior to recertification.
Maintain compassionate, culturally competent care across diverse patient populations.
Meet minimum required home visit standards based on level of care or arrange coverage with Team Manager.
Home Health Aide Supervision Responsibilities
Train, supervise and evaluate HHAs, homemakers and personal care workers.
Provide first-day phone orientation if not present in person and in-person follow-up within 24 hours.
Conduct supervision visits every 14 days.
Maintain an updated Plan of Care in the home and document HHA teaching in NetSmart.
Qualifications
Education:
Graduate of an accredited School of Nursing
BSN preferred
Experience:
Minimum 1 year Med-Surg experience
Community Health experience required
Hospice or Oncology experience preferred
Licenses & Skills:
Active NY RN License
Valid NYS Driver’s License
CHPN required after 1 year of hospice employment
Proficiency in MS Word, Excel and Windows applications
Strong assessment, prioritization and documentation skills
Excellent communication and customer service skills
Ability to work independently and use sound clinical judgment
Maintains confidentiality and professionalism
Demonstrates cultural sensitivity and respect for all backgrounds
