Employment Type:Full timeShift:Day ShiftDescription:Registered Nurse-Hospice- for Nursing Homes in the Saratoga RegionThe Community Hospice has a wonderful opportunity for a FT RN to work as part of our Hospice Care Team caring for patients and families in local Nursing Homes in the Saratoga Region.Workdays are Monday to Friday 8 am to 4:30 pm. NO WEEKENDS!Position Highlights:

NO WEEKENDS, NO ONCALL

National pre-taxed mileage reimbursement

Meetings and charting conducted via supplied lap top

What you will do:

As an RN you will case manage patients in the Nursing home setting

Responsibilities:

As an RN you will case manage patients in the Nursing home setting and respond to symptom management needs of our Hospice patients and coordinate with our team to provide added support to residents and families.

Collaborates with and builds strong working relationships with the nursing home team.

This is an ideal opportunity for someone who enjoys working closely with a team (our teams include Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers).

Training provided in end of life care

​What you will need:

A current license to practice as a Registered Nurse in the State of New York

Associates Degree in Nursing, BSN preferred

1 year experience as an RN , preferably in Long Term Care.

Previous hospice experience preferred but NOT required.

The RN must be able to communicate effectively

Valid Driver's License and Reliable vehicle

High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers

Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
