Unity House of Troy, Inc., a non-profit multi services agency dedicated to enhancing the lives of people in need has an opening for a SDOH Liaison in our Supported Housing program for those living with HIV/AIDS. This position has the option of a hybrid schedule. $750 sign on bonus for eligible candidates Responsibilities: Refer and link clients to transportation services that improve medical outcomes. Assist participants to access emergency financial assistance, transitional housing and other resources. Complete Social Determinants of Health assessments and other required program documentation. Coordinate services with other providers in the community. Provide referrals and linkages to care. Obtain, enter and track data for reports related to funding compliance. Promote a culture of inclusion and belonging. Practice a harm-reduction service philosophy. Requirements: Associate’s or Bachelor’s Degree in health, human, or educational services or equivalent. Three years experience working with those living with HIV/AIDS or other at-risk populations. Experience working with adults living with mental illness and/or substance abuse issues a plus! You must have a Valid and clean New York State Driver License and access to a reliable vehicle. Strong interpersonal, organizational skills, and experience/comfortability with computer systems and data entry required.
