Case Manager and RN Assessor (2)

Amazing Kids of NC, Inc. is a Human Services Agency that provides CAP/C Case Management Services. We currently have two part-time positions open that serve Wake, Durham, and Johnston Counties. (can go into full time later if interested) MUST RESIDE around geographical area. Must be a SW / Case Manager with experience working with special needs / medically fragile children and their families. Responsibilities include: completing assessments, developing a Plan of Care, collaborating with community providers,and conducting home visits. Positions can be a work from home position except for necessary trainings / meetings and home visits. RN Assessor position requires knowledge of medical diagnoses that affect children and young adults, completing assessments and incident reports, collaborating with other team members, and completing home visits.
