OverviewThis is a remote position. Only candidates in these states will be considered for hire: AZ, CA, CO, FL, IL, IN, KY, MI, ND, NE, OH, SC, TN, TX, WA, and WI.Team Coordinator –Virtual Support (TC)- Performs one or more assigned functions critical to the success of the Organization’s branch location(s) virtually including intake, health information data entry, medical records, scheduling. Contributes to the organizational success of providing high quality care by effectively coordinating location(s) office functions supporting patients, caregivers and employees. In addition, the virtual TC has a larger scope and impact and is responsible for all assigned workflow for designated locations, with assignments based on the needs of the Organization.Responsibilities

Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health information. Maintains confidentiality of patient information.

Works virtually and independently with limited direct supervision using organizational guidelines, policies and procedures and good judgment.

Functions as an effective team member and contributes to the achievement of organizational goals and objectives.

Demonstrates the ability to be efficient and productive by organizing job duties and responsibilities.

Completes assigned workflow tasks in a timely and accurate manner.

Assists with the running of various system reports.

Processes documents in a timely and accurate manner using organization standard document management software and processes.

Performs all necessary operational verifications to ensure compliance with regulatory, operational and financial requirements.

Assists in new hire orientation and training of new employees as requested by management.

Provides team coordinator support to location(s) as assigned; assignments may change daily.

Processes approved physician orders. Tracks documentation requiring a physician’s signature using organization standard document management software and processes. Escalates per organization standard process when signatures are not returned in a timely manner.

Responsible for timely processing of designated reports, summaries, and coordination notes to physicians and facilities required to comply with home health and/or hospice CoPs.

Ensures discharged files in EMR are complete.

QualificationsHigh School Diploma or equivalent.A minimum of two years customer service experience.Self-directed, detail oriented and organized.Excellent verbal and written communication skills.Strong interpersonal skills.Strong organizational and time management skills.Maintains confidentiality.Good computer skills, familiar with multiple software programs including Google Suite Programs.Pay Range$16.66 - $22.91 /hourWe are an equal opportunity/affirmative action employer.
