The Rehabilitation Registered Nurse is responsible for assisting patients in attaining and maintaining Amergisum function due to chronic illness and/or a debilitating injury. The Rehabilitation Registered Nurse assists patients in the recovery process, as well as creating a recovery plan with the ultimate goal of achieving an independent lifestyle as much as possible.Essential Duties and Responsibilities:
Assesses the physical, psychological, sociocultural, and spiritual dimensions of clients and their families, as well as their educational and discharge needs in order to formulate nursing diagnoses
Implements a plan of care by providing nursing care and education directly or through ancillary personnel
Coordinates educational activities
Utilizes appropriate resources to develop and implement an individualized teaching and discharge plan with clients and their families
Performs hands-on nursing care by utilizing the nursing process to achieve quality outcomes for clients
Provides direction and supervision of ancillary nursing personnel
Demonstrates professional judgment, uses problem-solving techniques, and delegates appropriately
Coordinates nursing care activities in collaboration with other members of the interdisciplinary rehabilitation team to facilitate the achievement of overall goals
Coordinates a holistic approach to meeting patient's medical, vocational, educational, and environmental needs
May facilitate community education regarding the acceptance of people with disabilities
Evaluates the nursing care that is being provided and modifies the plan, as needed, to achieve measurable goals and objectives
Minimum Requirements:
Current Registered Nurse License for the state in which the nurse practices
One year prior professional nursing experience in a rehabilitation setting preferred
CRRN certification may be required per state/client contract
Complies with all relevant professional standards of practice
Participation and completion of Amergis' Competency program when applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable to job site
Understand patient confidentiality and HIPAA requirements
Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required
Computer proficiency required
Must be at least 18 years of age
