To learn more go to www.sahpendleton.orgResponsibilitiesJob Summary:The Community Health Coordinator specializes in working directly with patients and patient groups to collaborate individualized integrated health care plans and follow up. Collaborates with healthcare provider offices the Emergency Departments hospital case managers and other community agencies/organizations to coordinate care and client needs. Works as a team member focused on care coordination. May be required to work non-traditional hours in order to accommodate care coordination.Essential Functions:

Conduct health education and screening outreach in the community with referral from the case management or health providers.

Reduce client barriers to health care and needed community resources and services.

Facilitate communication and coordination between community resources/services and providers.

Maintain knowledge and understanding of available community resources services and programs.

Create connections between vulnerable populations and needed community services and resources.

Assist with establishing a unified referral system as well as evaluate and improve the community health workers program.

Works closely with Providers and care coordination teams.

Help patients comply with health management plans and goals.

Ability to effectively communicate to staff and patients.

Thorough knowledge of the meaning and use of medical terminology and abbreviations.

Cultural Competence

Willing to become certified as a community health worker and maintain knowledge in the following areas: Medicare Oregon’s Health Insurance Marketplace (“Marketplace”) community resources and services dues/roles of community health worker/and or non-traditional home workers.

Demonstrate sound judgment and composure with the ability to take appropriate action in questionable or emergency situations.

Maintains a positive caring attitude towards patients.

Good computer skills using a medical management application.

Possess a strong work ethic and a high level of professionalism.

Complete when appropriate evaluations in the home.

Other duties as assigned by management.

Leads committees work groups and/or projects related to the implementation of educational activities

Facilitates use of technology to support all aspects of the program including the use of virtual methods

Develops processes that support the systematic and purposeful collection and dissemination of data and information

Allocates resources based upon internal and external environmental scanning

Cultivates a strategic partnership network that aligns with the system’s workforce plan and future needs

Aligns and ensures input is received from stakeholders across the system

Builds and maintains strategic internal partnerships (e.g. human resources quality improvement risk management Learning & Development national councils etc.)

Ensures a healthy multi-disciplinary learning environment across the system

Ensures a culture of inclusion and diversity

High school diploma or GED

Required Licensure and Certifications:

Community Health Worker Certification to be obtained within the first three (3) months of hire

Current AHA BLS Certification

Required Minimum Knowledge Skills and Abilities :

Language Ability:

Ability to write reports business correspondence and procedure manuals. Ability to effectively present information and respond to common inquiries or complaints from groups of managers clients customers and the general public.

Experience in program development implementation and evaluation

Emotional intelligence: Self-control self-awareness self-management social awareness

Ability to identify talent coach and develop others

Energy (capacity of acting being active positive force)

Math Ability:

Ability to add subtract multiply and divide in all units of measure using whole numbers common fractions and decimals. Ability to compare rate ratio and percent and to draw and interpret bar graphs.

Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written oral diagram or schedule form.

Computer Skills:

Must be proficient with the Microsoft Office Suite Google Internet navigation database management spreadsheet E.H.R. and graphic presentation packages.
