Hospice nursing is holistic and is focused on the individual client, integrating family/caregiver, environmental and community resources to promote an optimal level of client and family well-being.Essential Duties:

Assess and develops a plan of care through interviews of patient and primary caregivers/family, performs a physical assessment, integrates physical assessment with previous health and psychosocial history.

Provides case management and documents skilled care, treatments and diagnostic procedures.

Completes and documents initial evaluation of assigned patients. Assesses patient status and skilled nursing needs on admission and each subsequent visit.

Prepares recertification, discharge summaries and all federally required data entry and/or forms for Medicare.

Regularly evaluates the patient’s needs and informs physician and the supervisor of change of the patient’s condition, and provides necessary revision in the plan of treatment. Makes referrals as appropriate and authorized by physician. Coordinates services through consultation and interdisciplinary conferences.

QualificationsLicense/Certification:

Current RN license issued by the State of Oregon

Must have American Heart Association BLS for Health Care Providers within 30 days of hire

Must possess a valid Oregon Driver’s License, automobile insurance and provide own transportation

Experience:

1 year acute care nursing experience required, 2 years preferred

Recent hospice nursing experience within the last 2 months

Education:

Associate’s Degree in Nursing
