Job Title: Nurse Case ManagerJob DescriptionThe Nurse Case Manager is responsible for assisting in the development, planning, coordination, and administration of Utilization Review and Discharge Planning activities. This includes daily reviews of medical records to determine the appropriateness and medical necessity of admissions, continued hospital stays, and the use of ancillary services.Utilizing a collaborative approach, the Case Manager will assess, plan, implement, monitor, and evaluate the options and services required to meet an individual's health needs as advised by their provider. The role provides comprehensive, ongoing case management services to patients by coordinating and managing their care to meet multiple service needs across the continuum of care. The Case Manager ensures optimal patient outcomes that address quality, service, customer satisfaction, and cost-effectiveness. The Case Manager partners with the multidisciplinary team to establish care and allocate resources based on the patient's risk assessment. They assist patients and their families in coping with illness by optimizing self-care abilities and supporting consumer rights. Reporting to the Manager of Case Management and Care Transitions, the Nurse Case Manager I functions as a member of the clinical team under the guidance of the Nurse Case Manager Team Leader. This role coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge, managing the plan of care throughout the care continuum to ensure efficient patient movement through the hospital system. The Nurse Case Manager I also serves as a clinical expert in discharge planning, resource utilization, and case management.Hard Skills

Job TypeThis is a Contract-to-Hire position with a duration of 6 Month(s).Work SiteThis is a fully on-site position in Charleston, South Carolina.Work EnvironmentThe Nurse Case Manager will work with various types of patients including those in surgery, trauma, GI, and other specialized services. Each Case Manager will have 2-3 service teams, which are paired to work well together. The role does not require floating between different buildings; Currently, there is only a need for adult care management.
