Process transplant applications, complete phone intake screenings, obtain record and send referrals as needed to additional providers.

Responsible for receiving transplant applications from e-mail box, fax box, physician phone call, and mailbox, communicating any additional needs to the referring provider regarding the application, entering the application information into the electronic data base, calling the patient to obtain a detailed health history, and documenting this information in the electronic medical record.

Determines if there are any absolute or relative contraindications to moving the patient through the transplant evaluation process and partners with nurses before moving forward.

If appropriate, completes referral phases by scheduling patients for class, sending out welcome letter, requesting any additional testing that has already been completed at outside facilities, and follows up on requests to ensure documentation requested is received in a timely manner.

Asses any additional referrals related to the evaluation of the patient for transplant has been requested by the nurse, physician, or social worker, for follow up to ensure the referral was scheduled in a timely manner.

Ensures department receives documentation of evaluations from referral partners.

Build, maintain, and seek out strong relationships with referring providers, dialysis centers, and end stage renal disease/transplant patient and public community.

Serves as the main contact for referring physicians and dialysis centers by frequently visiting dialysis centers and referring physicians and providing them with our outreach materials (required by CMS).

Seeks to strengthen our relationship with the renal disease and transplant patient and public community by attending and representing our presence at events providing an opportunity to share our work with the community.

Responsible for sending out monthly outreach reports with updates specific to the dialysis center and referring physician of all active patients within our system that belong to that physician or dialysis center including referrals, evaluations, listed patients, and post transplants.

Survey physicians and dialysis centers, and patients regarding their satisfaction with our processes, how we could improve, what area we should focus on to ensure we are doing the best we can for that provider/patient, this person would be responsible for sharing the information obtained during these surveys with our quality team and implementing any readily available solutions.

Responsible for looking for additional opportunities for the program to participate in community events that support our patient population.

Maintain close working relationship with transplant coordinators to enhance throughput of transplant evaluations.

Works closely with THFW transplant coordinators and acts as a resource for the transplant coordinator to delegate tasks not requiring a transplant coordinator to complete (but are required for the patient to complete an evaluation) such as: sending referrals that the physician or transplant coordinator determines are necessary, printing/filling out required letters, and following up/obtaining dictations and test results on referrals/testing ordered for the patient, following up with patients who have not completed their scheduled tests, obtaining records from other places to prevent having to repeat testing, helping patients fill out all the needed paper work in the clinic, facilitating transplant orientation class.

Participate in quality and process improvement initiatives. Participates in monthly quality meetings to determine continuous improvement on current project.

Focuses on decreasing cancellation rates, by calling patients prior to their appointments to remind them of scheduled appointments.

As a transplant team member this person would be expected to participate and engage in the process as assigned.
