Position Responsibilities:

1. Client Activities

Data Entry

Receive all incoming Client referrals.

Maintain current lists of patients by payer classification.

Complete 2067s (Caseworker contact forms) as appropriate.

File documentation into client records.

Track admission status of clients hospitalized during service.

Document any complaints of dissatisfaction and report to Program Director.

2. Employee Activities

Notify RN of referrals for assessment.

Assign and ensure appropriate staffing for designated clients.

Assist with provider hiring.

Conduct attendant orientation.

Prepare client admission packets

Prepare and process employee applications

Serve as a back for field staff

Notify PAS Supervisor of supervisory visits two weeks prior to the due date.

Review field supervisors’ timesheets and EVV Report.

Ensure all service break reports are reported to the program director.

Obtain Authorization for services from health plan.

Verify and assist client in gaining eligibility for services.

Participate and perform in program audits and surveys as directed

3. Other

Serve On-call after hours and on weekends on a rotational basis.

Participate on the QI Committee if designated.

Perform QA reviews on client files.

Provide back-up for Administrative Assistant.

Provide attendant care as a back-up when required.

Perform all other or clerical duties as requested as related to PAS clients.

Answer incoming calls and direct caller as appropriate.

File incoming mail or faxes, and respond or forward as appropriate.

Send communications or faxes as required.

Interaction/Communication:

There will be heavy interface with

Staff

Key Management Personnel

Clients and their family members

Regulatory agencies

Representatives from payer sources.

Position Requirements:

At least 18 years of age.

Minimum of High School education or GED.

Valid Texas Drivers License.

Transportation.

Knowledge of using a personal computer.

1 year experience in home health, preferably in staffing.

Must be a fluent Spanish speaker

Types greater than 40 words per minute

Proficient in Microsoft

Physical Demands:

Moderate to heavy lifting may be required.

Moderate to longer periods of standing and walking may be required.

Moderate to heavy stress secondary to direct activity may be encountered.

Degree of Supervision Required:

Must be self- directed, motivated and goal oriented.
