Provide leadership and direction to a multi-disciplinary professional practice team. Develop coordinated care and service planning and evaluation and provide budget management, human resource management and assurance of compliance with regulatory and practice standards. Provide high quality, consumer-centered care and services by partnering and collaborating with other home care service managers, maternal and child nursing leadership and medical staff KFHPW-wide, clinic staff and multiple insurance providers. Essential Responsibilities: Provides leadership and direction to multi-disciplinary teams at multiple sites.

Assures consumer access to specific Home Care programs on a KFHPW-wide basis.

Ensures program complies with all state and federal licenser requirements.

Develops communication and coordination mechanisms to standardize service delivery, expectations, standards and program evaluation.

Represents nursing in team district, division and KFHPW-wide decision making forums.

Plans, implements and evaluates multi-disciplinary Home Care services at multiple sites. Authorizes appropriate staff development, implements and evaluates policies and procedures for the program in the service delivery area.

Develops and implements QA/QI programs.

Abides by sound fiscal management practices to assure appropriate stewardship of resources.

Develops, communicates and monitors productivity standards for staff and counsels as needed.

Plans geographic areas to allow maximum efficiency of staff and utilization of resources using innovative systems and programs to maximize patient care services within fiscal constraints.

Clarifies linkages between clinical functions, teams and utilization management.

Collaborates with other disciplines to develop innovative approaches to care which control utilization and costs and maintains or improves quality of care.

Assesses clinical practice needs and provides leadership and counseling for staff development to include orientation, continuing education and inservice plans.

Establishes care delivery systems able to change rapidly and respond to changing health care and customer demands.

Ensures compliance with patient care standards, state and federal regulations and goals and objectives of Home Care Services and KFHPW.

Basic Qualifications:

Experience

Minimum three (3) years of experience in health services management and successful work with teams including physicians at a middle management level and prior experience in program development and manager/agent of change.

Education

Bachelors degree in nursing.

License, Certification, Registration

Registered Nurse License (Washington) required at hire

Basic Life Support required at hire

Additional Requirements:

Clinical and business expertise, strategic planning, human resources and financial management.

Leadership and communication skills.

Preferred Qualifications:

Three (3) years of experience in managing care management teams.

Experience in care management principles to include utilization management and discharge planning.

Masters degree in nursing.

Knowledge of continuous quality improvement principles and tools, project management and Microsoft Works.
