Position Overview

We are seeking caring, reliable, and energetic individuals to join our team as Direct Care Providers / Personal Care Home Aides. In this role, you will support residents with activities of daily living and help maintain a safe, compassionate atmosphere where each person’s needs are respected.

-Key Responsibilities

As a Direct Care Provider at Fairfields’ Liberty Hall, you will:

-Resident Support & Care

Assist residents with Activities of Daily Living (ADLs) including bathing, dressing, grooming, toileting, and mobility support.

Monitor resident health by taking vital signs, observing for changes, and reporting findings accurately.

Support medication administration and help implement individualized care plans under supervision.

Prepare and serve nutritious meals, assist with eating as needed, and promote social interaction during meal times.

Safely use assistive devices and equipment to transfer and support residents.

-Safety & Environment

Maintain a clean, safe, and sanitary environment by performing regular cleaning tasks and following infection-control procedures.

Document care activities, resident progress, and any incidents accurately in accordance with HIPAA regulations and facility policies.

Team Collaboration

Work collaboratively with nursing staff, supervisors, and other care team members.

Attend training sessions and participate in resident-care planning as required.

-Minimum Requirements

To be successful in this role, you should meet the following:

-Education & Certification

High School Diploma or GED preferred (standard for direct care roles in PA).

First Aid and CPR certification is a plus (may be required by employer).

-Skills & Abilities

Strong interpersonal skills, patience, empathy, and the ability to engage respectfully with residents.

Ability to observe and report health changes accurately.

Comfortable with physical tasks including lifting, transferring, and assisting residents as needed (using proper techniques and equipment).

-Experience

Prior experience in assisted living, nursing homes, caregiver or home health settings is preferred but not always required.

Experience working with individuals with dementia, developmental disabilities, or memory challenges is beneficial.

-Other Requirements

Must be 18 years of age or older (typical for direct care positions).

Ability to pass background checks and any required clearances (e.g., ACT 33/34, FBI, state clearances).

Valid driver’s license may be required for certain assignments.
