Hospice of Humboldt is searching for a Social Worker for our Palliative Care department.
This position is benefited, 3/4-Time (32 hours a week). Schedule is Monday - Thursday.
The Social Worker provides counseling and medical social work case management to Palliative Care patients and their family/caregivers. This position assesses the social and emotional factors related to the patient’s illness and evaluates the psychosocial response to treatment and level of adjustment. This position interprets to the family, patient, physician, and staff, the social and emotional factors related to illness, financial and community resources.
Minimum Qualifications:
1. Master's degree in social work from an accredited college/university.
2. At least one (1) year of experience as a social worker in a health care setting.
3. Ability to work independently with a minimum of supervision.
4. Ability to listen and respond non-judgmentally with respect to other’s beliefs.
5. Ability to communicate effectively verbally and in writing.
6. Valid driver’s license, proof of acceptable automobile insurance coverage, and reliable transportation as specified by Hospice of Humboldt's policies.
7. Reliable mobile phone with plan that provides good local coverage.
8. Good computer skills. Proficiency in Microsoft Office applications and electronic medical record systems
