Our home health agency is looking for a Full-Time Care Coordinator to work in the office. Office Hours are Monday through Friday, 8:30 AM to 5:00 PM.
Job Responsibilities include (but are not limited to):
> Insurance Verifications
> Intake/Staffing
> Faxes/Emails
> Hospital Follow-up calls
> Schedule reminders
> Scanning & Uploading to patient charts
> Monitoring clinicians HR credentials
> Phone calls
Qualifications:
> Must be computer literate, and able to work well with computers/printers/scanners.
> Must be able to work well with others, especially in fast-paced environment.
> Must be able to multi-task.
> Must be organized, punctual, and able to read/write/speak English fluently.
Pay Rate based on prior home health experience:
Applicants without any prior experience in home health setting: Training $18/Hour, then reassess at end of 90 day probationary period.
Applicants with prior home health experience: Hourly rate to be determined upon interview.
