Transition Care Coordinator for Community Based Program

$29 - $32 per hour The Transition/ Care Coordinator is qualified in the human services field and possesses the skills and experience necessary to provide coordination and supportive services to program individuals with a mental health diagnosis and Intellectual/Developmental Disability (Dually Diagnosed). This is done in collaboration with the rest of the team and under the clinical supervision of the Program Director.Essential Duties and Responsibilities:+ + Provide recovery based, person centered services

Develop comprehensive goal plans in conjunction with individuals and family members.

Provide Service coordination, including coordinating and monitoring the activities of the individual’s treatment team.

Develop, write, implement, evaluate and revise, as necessary, individual’s treatment plans in collaboration with the Treatment Team.

Provide individual and group supportive interventions and symptom management ensuring that immediate changes in the treatment plan are made, as necessary.

Act as a liaison and consult with community agencies and families to maintain coordination in the treatment process.

Educate and support individual’s families and advocate for individual’s rights and preferences.

Conduct comprehensive assessments

Observes and reports patient progress and/or changes in patient status to physician and appropriate multi- disciplinary team members

Adheres to facility and Amergis policies and procedures at all times

Minimum Requirements:

Bachelor’s degree in Social Work, Psychology or Human Services Related field from a school of social work accredited by the Council on Social Work Education required

Complies with all relevant professional standards of practice

Participation and completion of Amergis’ Competency program when applicable

Current CPR if applicable

TB questionnaire, PPD or chest x-ray if applicable

Current Health certificate (per contract or state regulation)

Must meet all federal, state and local requirements

Successful completion of new hire training as applicable to job site

Understand patient confidentiality and HIPAA requirements

Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required

Computer proficiency required

Must be at least 18 years of age
