Care Coordinator / Case Manager (Part-Time – 3 hrs/day)
Location: San Diego (La Jolla area and Delmar
Hours: Part-time, approx. 3 hours per day
Compensation: (Insert rate)
We are seeking an experienced Care Coordinator or Case Manager to assist with senior fiduciary-abuse–related cases. The ideal candidate will have a background in elder care, social services, fiduciary oversight, or case management.
Responsibilities:
Monitor and document client wellbeing
Coordinate communication with fiduciaries, attorneys, and relevant professionals
Maintain accurate case notes and reports
Assist in resource navigation and follow-up actions
Uphold confidentiality and professional standards at all times
Qualifications:
Prior experience in case management, care coordination, or elder-abuse–related work
Strong communication and documentation skills
Reliability, professionalism, and discretion
Knowledge of elder services, APS processes, or fiduciary oversight is preferred
