Among Friends Healthcare is looking for Lead Care Manager / Community Health Worker for Santa Barbara / Lompoc area.
Qualifications:
Excellent interpersonal communication and outreach skills.
Close knowledge of and relationship with local community.
Ability to work independently with strong level of organizational skills.
BS degree in social work or related fields a plus.
Bilingual a plus.
Responsibilities:
Develops relationships with the community to educate and promote the Among Friends Enhanced Care Management program.
Develops and promotes referrals based on community relationships.
Identifies Medi-Cal recipients in need of clinical and non-clinical services.
Conducts an intake process and in-home assessment.
Identifies needed services through comprehensive assessment process.
Creates an ongoing care plan to address the participants’ needs.
Executes the care plan by providing needed services.
