We are seeking a compassionate and dedicated Caregiver / Individual Support Coordinator (ISC) to provide personalized support to an individual with intellectual disabilities in San Franciscoi. Your role will be vital in helping the client navigate daily routines, social activities, and personal development. The ideal candidate will be patient, adaptable, organized, and passionate about helping others live fulfilling and independent lives
Please note: This is not a live-in position.
Key Responsibilities:
Collaborate with client to create and implement personalized care plans
Facilitate social activities and encourage participation in community events
Assist with daily routines while promoting independence
Encourage skill development and personal growth through meaningful conversation
Offer gentle reminders and prompts for self-care tasks and medication
Monitor health and well-being while respecting independence
Communicate openly with other team members and family members to ensure coordinated care
Qualifications:
Experience supporting individuals with developmental disabilities preferred
Strong communication skills, with the ability to use clear and simple language
Patience and understanding when working with clients who may have difficulty communicating
Knowledge of person-centered care principles
Ability to work independently and as part of a team
Willingness to build supportive, respectful relationships with clients
Must have proper work documentation (no cash payments)
Hours of Operation:
Monday-Thursday 10pm-6am (overnight shift)
Monday-Friday 10am-3pm (day shift )
Monday-Friday 5.30pm-10pm (evening shift)
Rate $22- $24
