Program Manager 1B/My Place LA Transition Program Manager:The Program Manager 1B/My Place LA Transition Program Manager will assume responsibility for executing the program’s design, policies, and procedures. They will oversee the training and provision of technical assistance to MFP field coordination staff, enrolled OAAS Medicaid support coordinating providers, and providers affected by the transition, including direct service or Personal Care Providers. Additionally, they will facilitate the resolution of individual transition issues at the state program level. Ensuring data maintenance and developing reports on transitioning individuals to provide monthly and quarterly updates to the MFP Program Director will also be part of their responsibilities.The incumbent will demonstrate the following skillset characteristics:

Excellent communication skills, both written and verbal

Ability to think critically and creatively solve problems

Experience managing multiple assignments to completion

Skilled in meeting deadlines with ability to manage time effectively

Preferred Experience:

Experience securing housing for low-income individuals, including working with public housing authorities, landlords, and housing subsidy programs.

Experience in identifying, developing, and managing community resources, including partnerships with local service providers, non-profits, and government agencies.

Knowledge of long-term care services and supports, including home and community-based services (HCBS).

Knowledge of developing, negotiating, and managing contracts with service providers and vendors.
