Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our home health care team in a Per Diem capacity with availability to start with 15 visits a week and work every other weekend.
Availability to travel to surrounding towns of Chicopee, Holyoke, West Springfield is a MUST.
Availability to do am visits as early as 7:00am.
Pay Rate will be dependent upon experience.
Duties
-Administer medications and treatments as prescribed by physicians in the POC.
-Clean and change wound dressings.
-Monitor and record patient vital signs.
-Report any significant changes to the healthcare team.
-Provide education and support to patients and their families regarding health management and treatment options.
-Maintain accurate medical documentation using electronic health record systems.
-Complete all patient documentation accurately and timely.
-Ensure compliance with safety protocols and infection control measures.
Qualifications
-Home health experience a MUST!
-Current state licensure as a Licensed Practical Nurse (LPN).
-Current BLS/CPR certification.
-Strong communication skills with the ability to work effectively within a team environment is expected.
-Ability to perform clinical procedures safely and effectively.
