Home Care Direct Support Professional

Job Summary:

In this role, you will provide direct care and support to individuals with mental illness, brain injury, and/or developmental challenges in their homes, helping them achieve greater independence and an improved quality of life. At New Found Hope, We strive to take a person-centered approach by empowering employees and individuals served to collaborate on an individual basis to best meet the needs of those that we serve.

ICS staff play an important role in providing an extraordinary quality of care for individuals. ICS staff mainly provide individualized direct care services for 1 individual throughout the week. ICS staff build strong and professional relationships with these individuals, while providing the direct support services to help them lead more independent and fulfilling lives and to achieve their personal goals.

Responsibilities:

Providing supportive services documentation in accordance with individualized services plan – addendum (SP-A), and ensuring individuals served have satisfaction regarding services.

Providing support and training in community living. Support includes, skill maintenance, guidance, instruction, assistance with coordination of community living activities, and/or direct supervision. Training includes skill building and instructional services to acquire, retain and improve the person’s experience living in the community and must meet the individual’s identified needs. support to enhance the client’s well-being.

Support clients in developing skills to foster independence, including goal-setting and achieving personal milestones.

Follow care plans and work collaboratively with the guardian, caregivers, and healthcare professionals to ensure the best outcomes.

Ensure a safe, clean, and respectful living environment for the client.

Community participation Responsibilities:

Identifying and participating in community resources

Leisure, recreation, and socialization planning

Community and pedestrian safety

Interpersonal communications skills

Transporting Individuals served in personal vehicle

Health, safety and wellness Responsibilities:

Collaboration with the person to arrange health care (e.g., physical, mental, chemical), meaningful activities, social services, meetings and appointments

Health services support

Collaborate and create individualized strategies to support individuals to reach their health, safety and wellness goals

Adaptive skills Responsibilities:

Crisis prevention skills

Problem-solving

Support strategies for self-sufficiency

Implementation of positive support strategies to increase positive behavior, resulting in reduction or elimination of challenging behavior

Communicating with the supervisor concerning individuals served needs, progress, and changes in condition.

Documenting services provided and updating electronic records.

Adhering to professional standards, Life as Designed’s core values, following policies and procedures, and abiding by federal, state, and local regulations.

Other duties as assigned.

Job Types: Full-time, Part-time

Pay: $17.00 - $19.00 per hour

Expected hours: 15 – 30 per week

Qualifications:

High school diploma or equivalent (required).

Previous experience in caregiving, home care, or working with individuals with disabilities is preferred but not required; training will be provided.

Reliable transportation, a valid driver’s license, and proof of auto insurance (if applicable).

Strong interpersonal and communication skills.

Patience, empathy, and a genuine passion for helping others.
