Provides leadership and promotes collaboration with internal and external team members. Works cooperatively with primary care team(s) and support staff to provide direct patient care. Provides clinical expertise and mentoring for clinical staff. Supports the overall vision of patient center medical home by achieving a positive balance of healthcare quality, cost outcomes and patient satisfaction within specific teams. Accountable for the patient experience throughout the health care system. Ensures appropriate standards of care are implemented and in compliance with all state and federal regulations, standards of practice, corporate policies and procedures.Primary Responsibilities:

Foster a workplace culture that is consistent with the overall organizations and that emphasizes the identified mission, vision, guiding principles, and values of the organization

Provide direct patient care with exemplary customer service to internal and external customers. Duties included but not limited to rooming patients, screening patients; visit closure, patient ancillary visits (injection, wound care, blood pressure check); patient education; schedule follow up in the office or by telephone with patients for medical management of health condition

Assist with the coordination of patient care under the guidance of the primary care provider (lab testing, wound care, respiratory treatment, urgent or emergent referral, medication administration, patient form completion), act as liaison/facilitator in person or by telephone for health plan, case management activities, home health, shared medical appointments and disease management within scope of practice

Responsible for documentation of patient care in electronic medical record, including but not limited to office visit, vital signs, medication refills/updates, treatments, ancillary visits, patient communication (phone conversations, letters), referrals (new/updates/renewals) and patient tasking

Monitor back-office patient flow in collaboration with site management (RN, Team Leader or Provider). Communicate daily task assignments, staffing changes or shift in work requirements to help the team(s) complete work accurately and on time

Meet and follow applicable regulations and guidelines, to include assisting in the capture of data related to department quality measures, affordability measures, policies and procedures, state and federal regulations, and/or standards of care (e.g., Facility Quality Assurance, Huddle, Standard Work, Monthly Audits, Visit Closure Audits, Quarterly Back Office Audits, etc.)

Maintain, facilitate and promote primary care department protocols, processes, policies and standards to include but not limited to the following: Standard Rooming, visit close, exam room cleaning, 5S process, daily huddles, new and existing patient management, medication refills, pathology specimens, radiology procedures, and other specialty-specific protocols as signed by department head or designee

Maintain expert proficiency in computer-based health systems including but not limited to Automated Referrals, Touchworks and Allscripts Practice Management

Other duties as assigned (e.g., supply/medication management, ancillary appointments, outstanding referral management, patient de-escalation, patient complaints, daily duties, emergency equipment management and general patient/provider tasking)

Provides daily guidance, mentoring and development to clinic staff under guidance of RN supervision

High School diploma or equivalent

Active NV LPN license in good standing

5+ years of LPN experience

Preferred Qualifications:

Current state lab license, or ability to obtain within 30 days of hire

Current BLS, or ability to obtain within 30 days of hire

Proven charge, lead experience

Ambulatory experience

Adult medicine experience

Ability to multitask and lead others in a busy environment

Proven knowledge of complex LPN tasks. Able to navigate patient through the medical system to meet their needs. Demonstrated knowledge of (or ability to learn and apply) job functions

Demonstrated communication skills, excellent phone etiquette and customer service skills

Proven solid computer skills including familiarity with Microsoft Windows-based applications and organization skills

Working Conditions/Requirements: Normal clinic environment. Ability to pass respirator fit test and wear protective mask as deemed by OptumCare NV in the course of work at the clinic. Moderate to heavy phone and computer use. May float to other Southwest Medical (SMA) or Optum locations.Nevada Residents Only: The hourly range for this role is $23.22 to $45.43 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
