Position Summary Under the direction of the Director of Outreach the Outreach Representative will identify individuals and families in need of health care services throughout Franklin Township (Somerset) and school district and link them to the School Based Health Center. Outreach will be conducted throughout the Franklin School District, CBOs, churches, and local government and non-government agencies to increase awareness of SBHC services and increase referrals to SBHC and Somerville. Essential Functions, Duties and Responsibilities

Assists in obtaining essential support services such as Medicaid, food stamps, transportation, translation services and day care

Make appointments for children in the Franklin School District at the SBHC

Attend district events and meetings in order to raise awareness of SBHC services and refer parents and students to SBHC

Serves as an advocate for the patients, and acts as a liaison between the patients and community agencies

Attends in-service education and training programs

Provides basic health information and education to patients

Arranges referrals and provides transportation as needed

Assists medical staff in care management through outreach using patient registries

Provides ongoing social and emotional support through care management

Completes daily activity logs, referral forms and other reports, as necessary

Assumes other program related tasks as assigned

Knowledge, Skills and Abilities

Ability to communicate in English, both orally and in writing; ability to speak Spanish fluently

Ability to operate a motor vehicle and travel within the ZHC service site area

Strong interpersonal and communication skills, and the ability to demonstrate empathy

Ability to relate to patients as well as to local government and community organization workers and leaders

Ability to work independently, seeks assistance when necessary

Works with medical and social work professionals in a multi-disciplinary team, and works in a fast-paced work environment

Possesses basic knowledge of target populations’ ethnic and medical needs

Possesses flexibility, cultural sensitivity and tactfulness

Maintains HIPAA compliance in all regulated transactions and complies with all regulatory standards

As a condition of employment, we require that all staff are fully vaccinated against flu.
