Req Number 134822Job DescriptionProvides counseling services to bereaved family members of hospice patients and the general community.Job Responsibility1.Assesses bereaved clients and provides program orientation.2.Collaborates with healthcare team members to develop and implement client plan of care.3.Conducts individual, group and family counseling sessions.4.Serves as a liaison to referral sources to facilitate continuity of care.5.Performs related duties as required.
