The Rochester Regional Health Home Care team has provided compassionate, personalized care and assistance for adults and children for more than 50 years, and our quality of care has been among the top-ranked home care groups in the nation.STATUS: Full TimeLOCATION: Newark-Wayne Community HospitalDEPARTMENT: Home CareSCHEDULE: Monday-Friday, 8am-4:30pmATTRIBUTESRN Licensure in NYS requiredFive (5) years of experience, with two (2) years of experience in the home care setting, preferredExperience in working in partnership with the interdisciplinary healthcare team Excellent interpersonal written and verbal communication skillsExperience utilizing an Electronic Medical Record and Microsoft officeDemonstrated ability to apply decisive, sound, objective clinical judgment for cases with varying complexityRESPONSIBILITIESEvaluation & Collaboration. Evaluate eligibility, needs and care resources for patient; obtain insurance information and authorizations for appropriate reimbursement; collaborate with facility care coordinators, social workers and physicians to find solutions for challenging patients requiring home care Assessment & Development. Complete a comprehensive clinical assessment for home care services and develop a home care plan; coordinate all in-home services, including professional, paraprofessional services, durable medical supplies and equipment Documentation. Complete all required documentation in patient's facility record; gather and analyze data and complete all assigned reportsEDUCATION: AS: Nursing (Required)PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.LICENSES/CERTIFICATIONS: BLS - Basic Life Support - American Heart Association (AHA)RN - Registered Nurse - New York State Education Department (NYSED)PAY RANGE: $70,700.00 - $94,300.00The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future.
