CNA - Job Description

Provide skilled care to the Veterans who reside at the Home, such as but not limited to:

Resident Care Assist residents with Activities of Daily Living (ADLs): bathing, dressing, grooming, oral care, and toileting.

Provide mobility support: transfers, ambulation, and use of assistive devices (walkers, wheelchairs, lifts).

Assist with feeding, including setting up trays, feeding residents when needed, and documenting intake/output.

Ensure residents’ comfort by making beds, changing linens, and keeping rooms clean and safe.

Provide perineal care and maintain resident hygiene.

Observation & Reporting Monitor residents’ physical and emotional status, and promptly report any changes (e.g., pain, skin breakdown, behavior changes) to the nurse.

Measure and record vital signs (temperature, pulse, respiration, blood pressure, weight, height, and oxygen saturation as directed).Track and report food and fluid intake and output.

Observe infection control protocols (hand washing, PPE use, isolation procedures).Communication & Documentation

Document all care provided in daily logs or electronic medical records as required.

Communicate resident needs and changes in condition to the nurse in a timely manner.

Participate in shift hand-off reports for continuity of care.

Resident Safety Maintain safe environment: prevent falls, ensure call bells are within reach, and respond promptly to alarms. Apply safety devices (e.g., gait belts, alarms) as directed by nursing staff. Follow facility protocols for handling emergencies such as falls, elopements, or fire drills.

Supportive & , emotional support, and reassurance to residents respectfully .

Encourage independence while assisting with care.

Respect and uphold residents’ rights, dignity, and privacy at all times.

Assist residents with participation in recreational and social activities.

Teamwork & Collaboration Work under the supervision of an RN or LPN, following directions carefully. Assist nurses with non-invasive clinical tasks as delegated (e.g., collecting urine/stool samples).Support colleagues by answering call bells and assisting with residents beyond personal assignments when needed

May accompany the transport of residents to outside appointments.

Please note that the employees would work on holidays only by request of the employer and would only be straight pay. Employees work overtime only at the request of the employer and get paid at overtime rate.

LPN - Job Description

LPN Tasks and Duties in Facility( Provide care and service to minimum of 20 and maximum of 40 Veterans)

Supervise C.N.A in Resident Care Provide direct patient care, including bathing, dressing, grooming, and toileting assistance as needed.

Monitor residents’ physical and emotional well-being, reporting any changes to the RN or physician.

Assist residents with mobility, transfers, and ambulation, ensuring safety and use of assertive devices.

Clinical Responsibilities Perform vital signs, weight checks, and assist C.N.A with ADL care such as feeding weight measurement

Administer medications (oral, topical, injectable, inhaled) as prescribed and document accurately.

Provide wound care, including dressing changes, skin assessments, and preventive skin care.

Collect specimens (urine, stool, sputum, blood via finger stick) for lab testing.

Monitor and report lab values and diagnostic results to the RN or physician.

Insert, maintain, and monitor urinary catheters, IVs (depending on state regulations), and feeding tubes.

Administer oxygen therapy and monitor residents on oxygen.

Communication & Documentation Document all care provided, observations, and resident changes in the medical record promptly.

Communicate effectively with the RN, & Providers, residents, and designated representative within scope of LPN

Report resident status changes of change in condition immediately to the supervising RN or physician.

Participate in shift hand off reports to ensure continuity of care.

Supervision & Delegation Supervise and direct CNAs in daily resident care tasks.

Ensure CNAs follow care plans and report concerns.

Perform skin observation during shower and pre/post dialysis report and document any abnormality to RN

Provide feedback, support, and guidance to C.N.As

Care Planning Assist in reviewing, and updating resident care plans under RN supervision.

Participate in care conferences with interdisciplinary team members.

Implement interventions in accordance with care plans and physician orders.

Emergency Response Recognize and respond promptly to medical emergencies with RN prompt notification for assessment (falls, seizures, cardiac/respiratory distress).

Assist in Perform CPR and first aid as required.

Contact RN, physician, or emergency services when necessary.

Resident Advocacy & Support Promote dignity, respect, and resident rights in all care activities.

Provide emotional support to residents and families.

Educate residents and families about health needs, medications, and care routines (within LPN scope).
