Mental Health Case Manager

Job Summary: Working within a multi-disciplinary team, will provide health care coordination services for person living with HIV/AIDS, and LGBTQ population with focus on patients with a mental health diagnosis. Position will ensure retention of clients in medical and mental health care by advocating for identified and needed services, addressing barriers to care, and ensuring the maintenance of on going services.
