Founded in 1837, Cuyahoga County’s safety-net health system operates four hospitals, four emergency departments and more than 20 health centers.Summary:Under general supervision, advocates for individuals and families by promoting health, reducing disparities, improving access to healthcare, and reducing social drivers of health. Services performed include outreaching out to individuals, families, and community members to identify social needs. Connecting with identified at-risk individuals, families, and community members to care, services, and resources. Builds partnerships within MetroHealth System and with local community organizations. Understands the socio-economic needs and culture of the populations/communities served to identify barriers to individual health and to advocate for changes to improve community healthQualifications:Required:

High school diploma or passage of a high school equivalency exam.

Must obtain Community Health Worker certification from an accredited program within one year of hire.

One year of experience working with clients in a community-based setting.

Life experiences reflective of the community/target population served.

Reliable transportation to travel to designated community locations.

Ability to work with diverse client populations, community organizations, and staff at all levels.

Ability to work independently and collaboratively within a team from general instruction.

Excellent verbal and written communication skills.

Outstanding interpersonal, administrative, and organizational skills.

Highly skilled in using personal computers, including, but not limited to, Microsoft Office applications.

Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds.Program-Specific Requirements:

School Health Program: Experience with school-aged children and families

Obstetric Program: Experience with a pregnant woman

Opportunity Center at Via Sana: Bilingual OR multilingual OR Spanish speaking

General Institute for H.O.P.E. – Experience with adults with chronic health conditionsPreferred:

Active Community Health Worker (CHW) certification from an accredited program

Chronic Disease Self-Management Coaching Certification

Additional coaching or certifications relevant to the population served

Bilingual
