Job Title: PharmacistJob DescriptionWe are seeking a dedicated Pharmacist to join our team. The ideal candidate will evaluate physician medication orders for appropriateness, make therapy change recommendations, maintain patient databases, ensure appropriate laboratory tests, and collaborate with the healthcare team. You will document communications, implement safe drug policies, inform the Pharmacy Manager of errors, assist staff with procedures, and perform other assigned tasks.Responsibilities

Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.

Make recommendations for therapy changes to physicians and facility staff.

Maintain patient/resident database, including current and past medical history, clinical and laboratory data, and concurrent medication.

Ensure appropriate laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with healthcare team members.

Collaborate with the healthcare team to provide clinical and technical support.

Document all communication with healthcare team members, patients/residents, and their families in the medical record.

Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.

Inform the Pharmacy Manager of medication-related errors.

Assist facility/agency staff with procedures related to safe and effective drug procurement, maintenance, administration, and disposal.

Perform other tasks as assigned.

Essential Skills

Bachelor's Degree in Pharmacy; Pharm D preferred.

One year of experience in long-term care preferred.

Tennessee Pharmacist License.

Work EnvironmentThis is a NON-RETAIL, closed-door pharmacy environment where you can focus on fulfilling the pharmaceutical needs of our long-term care and senior living clients. The shift is from 10am to 7pm, Monday to Friday, with possible weekend shifts.
