Job Description:This position is accountable for the provision of patient care and other activities as permitted by licensure and defined within Credentialing and Privileging documentation (hospital), Credentialing and Scope of Practice (Medical Group), Delegation of Services Agreement (DOPL) or other similar documentation as appropriate. As part of that patient care, this position is accountable to deliver optimal patient care through effective clinical practice; education of the patient, patient's family, clinical staff, and others; consultation with other healthcare providers; and maintain current knowledge regarding options for optimal patient care and outcomes.This role will provide appropriate triage and care coordination for patients with serious health issues or who require an alternate level of care. Will provide care to ease pain symptoms caused by different conditions. The role will work almost exclusively in a telehealth setting in Connect Care-Pain Management Services, seeing patients for both new evaluations and providing follow-up care, medication management, and appropriate referral management.Essential Functions

Perform patient assessment to obtain a complete and accurate record of the patient's clinical information and medical symptoms.

Order tests to obtain additional information needed to ensure proper and complete care of patient and aligned with Intermountain Health’s standards as applicable.

Perform diagnostic and/or therapeutic procedural care as defined by Scope of Practice.

Determine a plan of care according to Intermountain guidelines.

Appropriately triage, treat and coordinate the care of patients presenting for Telehealth evaluation consistent with appropriate prescriptive practices, Intermountain Health guidelines, and accepted standards of care.

Assess patient to identify opportunities to coordinate, collaborate and integrate care within the broader Intermountain network and community.

Understands performance, proper use, and documentation of CLIA-waived point-of-care testing and Telehealth practices and equipment.

Document clinical care in an appropriate and timely manner. Code and bill accurately per established protocol and procedure and ensure that records are detailed and complete.

Stay abreast of current trends in telehealth and healthcare practices and participate in continuing medical education to ensure optimal clinical knowledge and effective patient care skill.

Assist in education/promotion of Connect Care/telehealth technology, clinical application, and proper use to patients and Intermountain clinicians and staff.

Conduct, coordinate, and promote education to increase the knowledge of patients, patient family, and others. Provide appropriate instruction based on patient needs to support the continuum of care and/or medical home.

Participate in research and utilize evidence-based practice to promote quality patient care, protocol development, care process model development, and best practice standards.

Assist in the planning, development, implementation, and coordination of Telehealth activities (potentially including making recommendations for program changes, development/implementation/coordination of new clinical areas, development of outcome and quality measures.

May be required to occasionally perform provider duties in a clinic setting to ensure appropriate staffing.

Qualifications

Physician Assistant or Advanced Practice Registered Nurse degree from an accredited institution is required. Education will be verified.

Current licensure as a Physician Assistant (PA) or Advanced Practice Registered Nurse (APRN) within the state(s) of practice is required. Eligibility for licensure in Utah, Montana, Colorado, and Nevada.

Eligible for State Controlled Substances License II through V is required.

Eligible for Federal DEA Certificate II through V is required.

Basic Life Support (BLS) certification for healthcare providers is required.

4+ years of experience as a PA/APRN is preferred.

Prior experience in telehealth or similar technology is preferred.

Bilingual in Spanish and English is preferred.

Physical Requirements:Physical Requirements

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.

Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.

Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)

May be expected to stand in a stationary position for an extended period.

For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing, and reading signs, traffic signals, and other vehicles.
