CNA-Home Health/Hospice

Provides direct patient services that are physician ordered. Provides hands-on personal care and support services under the direction and supervision of a RN.Provides assistance to patients with ambulation and safe transfers per agency policies. Reports and condition changes to Case Manager and documents any services provided and changes noted in patients condition.Participates in care coordination meetings per company policy.Must be able to lift up to 50 lbs and assist with transfers of patients.Must be able to drive around Metroplex area to patients residence to provide care.Must respect patients home and environment.Must keep up required continuing education requirements for a CNA.
