With our combined resources CommonSpirit is committed to building healthy communities advocating for those who are poor and vulnerable and innovating how and where healing can happen both inside our hospitals and out in the community.ResponsibilitiesJOB SUMMARY / PURPOSEThe RN Program Coordinator over our Stroke and Stemi programs will develop strategies, coordinate day to day operations, and evaluate the impact of the services provided to patients. Works with interprofessional team to facilitate overall programmatic growth.ESSENTIAL KEY JOB RESPONSIBILITIES

Maintains the daily operations of the program.

Coordinates care throughout the program utilizing appropriate tools to ensure high quality, cost effective care is

provided to all patients referred to the program.

Performs preliminary assessments and data collection.

Acts as a resource person and liaises with others on matters related to the program and the program’s services.

Partners with other clinical team members in assessing and supporting the needs of the patients and families.

Presents program data and outcomes to internal and external audiences.

Provides educational offerings to internal and external audiences on program services.

Collaborates with management on the expansion of program offerings and services.

Maintains program records and files including documentation in electronic medical record as appropriate.

Performs housewide education programmatic.

QualificationsRequired:Bachelors DegreeRegistered Nurse (RN)Three years program experienceDesired:

Excellent verbal, written and interpersonal communication skills

Ability to educate and deliver presentations

Critical care nursing experience
