Our CompanyAbode Care Partners OverviewAbode Care Partners is seeking a dedicated and dynamic individual to join our team as Community Liaison. In this role, you will be responsible for engaging with the community and promoting our patient centered, comprehensive and integrated primary care services. Reporting to the Regional Administrator, you will play a vital role in analyzing market potential, developing marketing strategies, and fostering trust with health care professionals and communities to drive patient referrals and increase lives served. The community liaison will also facilitate the discovery of innovative solutions to strengthen engagement, growth, and improvement in core quality metrics for a geographic region. Responsibilities
Analyze the potential of our service area to identify target markets and opportunities for growth
Establish and nurture professional relationships with the key staff at assisted living communities, senior care professionals and other senior support referral sources
Respond immediately to all service inquiries from partnered communities
Maintain open and effective communication with clients, family members, facility contacts, business colleagues and Providers
Complete intake process for new admissions via Electronic Medical Record to include securing consents, completing the voluntary attribution process, and any relevant assessments
Support growth targets through relationship management and patient enrollment efforts
Weekly face to face contact within established communities as well as potential communities
Actively participate in weekly virtual meetings with Regional Team to discuss opportunities, goals, growth, and quality
Partner with communities/facilities and other healthcare professionals to host informational and educational events
Qualifications
Bachelor’s degree in related field
Previous experience in related field – healthcare sales, business development, or healthcare support
Strong interpersonal and communication skills
Computer literacy and knowledge of relevant healthcare and administrative software
Excellent analytical and problem-solving skills
Organizational and time management skills
Ability to work independently
Ability to work closely and professionally with others at all levels of the organization and communities that we serve
Demonstrates the highest degree of customer service and professionalism
Travel up to 75%
